Please upload your completed "Superannuation (super) standard Choice form".
This is located on the ATO website. *

Please upload your superannuation fund’s “Letter of Compliance”. This letter can be found on the superannuation fund’s website. *

In order for us to make your superannuation remediation payment we need to verify your ID.
Please upload certified copies of two of the following documents: Passport, Driver Licence, Birth Certificate. *
