The Professional Certificate in Coordinated Care is delivered in a series of structured modules allowing health professionals to work flexibly and build towards their professional certificate.
There is potential for clinicians and members of the LHD to use this Professional Certificate as a viable and appealing option for the annual required Continuing Professional Development (CPD) hours.

Introducing Coordinated Care
This course delves into the essentials of collaborative, holistic, and person-centered care. Explore health, delve into social determinants, and learn how to identify and manage the needs of at-risk individuals and groups. Additionally, gain insights into risk management, handling competing demands, and addressing clinical complexities.

Coordinating Person-Centred Care
This course equips healthcare professionals with the skills to identify, assess, and monitor the influential factors—social, behavioral, and environmental—that impact health outcomes in clients, families, carers, and communities. Additionally, learn to construct and defend person-centered, goal-directed, holistic, effective, safe, integrated, and efficient care plans tailored to organizational needs. Gain the ability to proactively advocate for clients and foster collaboration with fellow service providers.

Coordinated Care in Practice
This course delves into the behaviors, attitudes, values, and norms that either facilitate or challenge the roles and responsibilities of care coordinators.
