Clinical Placement Requirements 2013
In order for students to attend a clinical placement in a NSW Ministry of Health Facility student must meet the following criteria:
• undergo a Criminal Record Check and receive a National Police Certificate (from NSW or Australian Federal Police)
• a signed NSW Health Code of Conduct Agreement for Students Undertaking Clinical Placements
• a signed NSW Health Student Declaration (to fulfil requirements of the Commission for Children & Young People Act 1998)
• meet immunisation requirements
• completed NSW Health Form 2. – Tuberculosis (TB) assessment tool
• completed NSW Health Form 3. – Student Undertaking/Declaration

How Important Is This?
These requirements are VERY important!!
All students undertaking a Clinical Placement in a NSW Health Facility must meet ALL criteria. Any student who does not meet ALL criteria will not be allocated a placement. If you are unable to complete Clinical Placement you will not have met the requirements to graduate from your chosen degree.
Verification must be received before you attend placement. Students who are not verified at least 7 days prior to day 1 of placement will not be allocated a placement.
Faculty of Health Students requiring compliance include:
• Bachelor of Medical Radiation Science (Diagnostic Radiography)
• Bachelor of Medical Radiation Science (Nuclear Medicine)
• Bachelor of Medical Radiation Science (Radiation Therapy)
• Bachelor of Medicine
• Bachelor of Midwifery
• Bachelor of Nursing
• Bachelor of Nutrition and Dietetics
• Bachelor of Occupational Therapy
• Bachelor of Oral Health
• Bachelor of Physiotherapy
• Bachelor of Podiatry
• Graduate Diploma of Dental Therapy
• Master of Pharmacy
How to Comply with NSW Health Requirements
Verification is received from NSW Health staff only when a student can provide all necessary evidence. There are different procedures for validating the various required components so be sure to read the following information carefully.
Criminal Record Check
This requirement is part of the NSW Ministry of Health's duty of care to patients and clients receiving health care in NSW and falls under the NSW Ministry of Health Employment Screening Policy [PD2008_029] (http://www.health.nsw.gov.au/policies/PD/2008/PD2008_029.html).
A National Police Certificate (NPC) can be obtained from either the NSW Police or Australian Federal Police. Both certificates are printed on secure paper (which has a number of security features) used by all Australian Police Services.

National Police Checks must be obtained at the student's expense. Costs may vary and it is advisable to check both options for best price.
N.B Any other criminal record checks are not accepted by NSW Health.
IMPORTANT: the name appearing on the Certificate must exactly match that displayed on your student identification card. If they do not match the Certificate will not be accepted by the NSW Ministry of Health. If your name changes during the course of your degree you will need to obtain and submit a new NPC.
International Students
International students also require the following:
• Overseas Police Certificate from all countries of residence (with English translation)
or
• Statutory Declaration stating you have/do not have criminal convictions/pending charges from your home country or any country that you have resided in. If you do have such records, you must provide details, listing the offences, the dates and any court outcomes. Where convictions/charges exist you must follow the instructions under heading What if I have any prior convictions or are convicted during enrolment? (above).

Immunisation/Vaccination
Specific immunisation requirements are outlined in the NSW Health Policy Directive (Document No: PD 2011_005): Occupational Assessment, Screening and Vaccination against Specified Infectious Diseases which can be found at: http://www.health.nsw.gov.au/policies/PD/2011/PD2011_005.html.
What Immunisations Do I Need?
You need to provide NSW Health with evidence that you are immune to a range of diseases. This may require you receiving additional immunisations to those you already have.
The summary of what immunity and evidence you need is available at: http://www.health.nsw.gov.au/...checklist.pdf. It is advisable that you print one of these documents and take it to your GP to discuss what you need to do.
All immunisations, serology reports etc., must be obtained at the student’s expense. Some immunisations are provided free of charge by the Commonwealth Government so be sure to ask your GP if you are eligible.
Immunisation courses can require multiple treatments and can take up to 6 months to complete. It is a NSW Health directive that immunisations and serology (blood tests) must be completed within 6 months of course commencement. So get started NOW!!

What do I need to do for NSW Health?
You need to provide a number of documents to NSW Health in order for your disease immunity to be assessed:
