Required medical information


Note: The NSW Health Verification process can take up to 6 months to complete. This has to be completed within the first year of study. To ensure your placement doesn’t get pushed back please start this verification process as soon as possible.


There are two sections to complete within the required medical information tab. Make sure you read through each section carefully before moving on to the personal requirements tab.

Section One: Vaccinations

All students must be fully vaccinated against the following diseases. Not being fully vaccinated may delay your placement.

In some cases, ‘temporary’ NSW Health compliance may be allowed whilst you obtain booster / additional vaccinations. You can read about temporary compliance in the Key Information section.

Hepatitis B

3 paediatric doses; or

2 adults doses at high school (11-15 years); or

3 adult doses for >20 years of age.

Serology must be taken at least 4 weeks after the last dose of Hepatitis B.

Immunisation evidence and a Hepatitis B serology report confirming immunity status.

Hepatitis B Vaccination Declaration

Hepatitis B Vaccination Declaration (HBVD) witnessed by an approved assessor. ONLY if all attempts fail to obtain vaccinations records, international student immunisation histories will be accepted if translated into English.

A Hepatitis B serology result of anti-HBs level >10 mlU/ml showing immunity is required before a HBVD can be completed.

Hepatitis B serology report confirming immunity status and witnessed HBVD.

Diphtheria, Tetanus and Pertussis (dTpa)

1 Adult dose of dTpa in the last 10 years.

Serology is not acceptable evidence.

Immunisation history statement (myGov).

Measles, Mumps and Rubella (MMR)

2 doses of MMR vaccine at least one month apart.

Can be used as evidence of immunity.

Immunisation history statement (myGov); or

Positive IgG for measles, mumps, and rubella; or

Born before 1966.

Varicella (chickenpox)

2 doses of the varicella vaccine at least one month apart; or

1 dose if vaccinated before the age of 14.

Can be used as evidence of immunity.

Immunisation history statement (myGov); or

positive IgG for chickenpox; or

History of chicken pox recorded by a doctor on the Australian Immunisation Register (AIR).

COVID-19

Vaccination is strongly recommended to remain up to date with COVID-19 vaccinations as recommended for your age and health status by the Australian Immunisation Handbook.

Influenza

1 dose of the current southern hemisphere seasonal influenza vaccine by 1 June each year.

Serology is not acceptable evidence.

Immunisation history statement (myGov)

Submitted by May annually.


Where can I get evidence of my vaccinations?

You must provide evidence of complying with immunisation requirements.

This can be done by using your Immunisation History Statement, which you can access on myGov.

Some vaccinations require or use serology reports as evidence. You can access serology reports through your GP.


What if I was vaccinated overseas?

The Vaccination Record Card for Category A Workers and Students is to be completed by a GP if you were vaccinated overseas and:

  1. do not have evidence of vaccination on the Australian Immunisation Register (AIR)
  2. do not have a record of your immunisations
  3. do not have English translated overseas vaccination records
  4. you only have some evidence of vaccination

Note: Vaccination records recorded in a foreign language may be translated using the Free Translating Service provided by the Department of Home Affairs or using a local translation service.

Of any country where you have lived for more than six months since turning 18 must be provided (along with an official English translation if not already in English).

Where do I get my police check done?

Your NCRC must be obtained through an ACIC Accredited Body or an Australian State or Territory Police Force (e.g., NSW Police or the Australian Federal Police).

For detailed information—including guidance for those currently employed by NSW Health, international students, or those completing an AFP NCRC—please refer to the Key Information page.

You must complete an International Police Check. If you are unable to provide an International Police Check, complete an Overseas Statutory Declaration instead. If completing the Overseas Statutory Declaration you must write your full name, address and occupation. Read the first paragraph of the declaration and cross out the text that does not apply to you; I do not have / have criminal convictions/pending charges. The authorised witness must check you have fully completed the declaration and cross out the text that does not apply in both sections 1 and 2.

4. Submission

Submission requirements


If your submission is incomplete or incorrect it will be returned to you. To reduce the risk of your documentation being returned to you, ensure you submit only after you have fully and correctly completed all the verification requirements in your first submission.


It is important that you read through these steps carefully, as an incorrect submission will result in delays to the compliance assessment.

You will need to create one PDF document which includes the following documents. Please read through the list carefully to assure you have included all documents.

  1. A scanned colour copy of your current University of Newcastle Student ID Card
  2. A colour copy of your National Criminal Record Check (NCRC)
  3. International Police Check
  4. Signed NSW Health Code of Conduct Agreement for Students
  5. Evidence of complying with immunisation requirements, including an Immunisation History Statement, serology reports, and (if applicable) a Vaccination Record Card for Category A Workers and Students
    Immunisation requirements include immunity against the following diseases:
    1. Diphtheria
    2. Tetanus
    3. Pertussis
    4. Hepatitis B
    5. Measles
    6. Mumps
    7. Rubella
    8. Varicella

Bulk Verification Days

Students enrolled at Gosford and Ourimbah campuses must attend a Bulk Verification Day (BVD) to submit their NSW Health documentation to the Central Coast Local Health District (CCLHD) team.

Submission steps

  1. Ensure you have all of the required documents. If anything is missing your verification will be significantly delayed
  2. You will need to register for the next Bulk Verification Day.
  3. You will be required to bring hard copies of your documents on the day you attend.  If you do not have all your documents on the day you attend, your appointment will be cancelled and you will be required to re-book for another date and time when you have all your documents available.

IMPORTANT: You should keep all of your original documents (including serology reports) and certificates in a safe place. You should take them with you on your first day of each placement as the facility has the right to request to see them.




Updating your verification?

If you have previously been verified and need to update your documentation, please follow the steps below:

  1. Create a single PDF which includes the relevant updated completed forms. The first page of your PDF should be your Student ID.
  2. Name the PDF as FirstName_LastName_Student ID
  3. Send an email to the CCLHD verification team. Please make your subject line clear, for example:  Joe_Smith_c3123456 (Blood Borne Virus verification update)
