Need help with workplace requirements?
Who needs to complete the Blood Borne Virus Declaration (Section 4)?
The Blood Borne Virus Declaration, included within the Undertaking/Declaration Form, is only required for Medicine, Midwifery, and Oral Health students. Students enrolled in other programs do not need to complete Section 4.
