Biography

Associate Professor Denise Blanchard is the Head of Discipline Nursing based at the Central Coast Clinical School, Gosford. Denise is a skilled nursing and social science researcher with leadership in education and health, health management and simulation development. Denise's expertise extends across the dual areas of mental health and psychological well-being for older people. Her research is in aged care outcomes, which relates explicitly to dementia and aging in place and long term conditions. In addition, Denise has led and contributed to developments in nursing education through curriculum, simulation development and the translation of evidence. Denise's leadership role provides strategic leadership about nursing to the broader University and engages nationally and internationally. Denise has actively maintained practice and nursing in Australia and New Zealand. Denise is research-active, and her research interests include, Aged care nursing and aging in place, Psychological well-being related to long-term conditions, Simulation and Competency-based education and training. Denise is driven by her belief that education is an opportunity to support students to achieve their aspirational wishes to become a part of the nursing profession. Also, she recognises that nurses bring considerable expertise to population health and well-being.

This chapter addresses the issues of gender and its relationship to leadership for health professionals and how different generational groups may or may not influence leadership. It considers the potential causes of gender differences and the barriers women may face in reaching their leadership potential, offering recommendations to help women overcome the challenges. The chapter focuses on the issue of generational differences and considers whether such differences affect how health service leaders lead or manage. Gender inequality remains evident in leadership roles worldwide. There is considerable agreement that women face more barriers to becoming leaders and more pressure from their roles than men do, especially for leadership roles in male-dominated professions or industries. Women are becoming more prominent in senior leadership and middle management positions in a range of organisations. COVID-19 will also impact upon their formative years through home education and disruptions to face-to-face personal connections.

Introduction: Australians in rural areas experience limited access to services and poorer health outcomes than residents of metropolitan areas. Nurse practitioners (NPs) were introduced in 2000 to reduce pressure on the health system, address workforce shortages and improve rural populations' access to health care services. Objective: This scoping review sought to identify, examine and synthesise research evidence of NP practice in Australian rural primary health care services to better understand how NPs are addressing service gaps in rural areas and to identify existing gaps in our knowledge. Design: Peer-reviewed and grey literature from July 2012 to June 2022 was accessed from seven electronic databases, grey literature and hand searching of reference lists and citations. Findings: From 154 articles, 19 articles of relevance were identified. Several projects describe the processes required for success, whilst others reported the challenges and barriers encountered. Limited research evidence of rural NP practice exists with a significant gap about how roles operate and their value in primary health care. Discussion: Uptake and envisaged benefits of rural primary health care NP roles have yet to be realised, with barriers to implementing and sustaining NP roles persisting. Low-level awareness with ambiguity at health service and community level adversely impact on systematic implementation of NP roles. Conclusions: Robust evaluations demonstrating the value of NP skills and practice are needed in combination with bipartisan support from all levels of health care and government providing adequate funding to enable effective implementation of NP roles in poorly resourced rural areas.

Background Communication difficulties, including hypokinetic dysarthria and swallowing difficulties (dysphagia), affect a large percentage of people diagnosed with Parkinson¿s disease. Onset of these symptoms has been identified in up to 78% of people with early-stage Parkinson¿s disease. Communication difficulties are frequently disregarded until they have a significant impact on quality of life, while the person may often be unaware of indicators of dysphagia and the associated risk of aspiration pneumonia. Objective The aim of this article is to increase awareness of the importance of identifying and addressing the communication and swallowing difficulties experienced by people living with Parkinson¿s disease. Discussion Early identification, regular review and monitoring enable the clinician to support the implementation of evidence-based, effective interventions. Collaboration with the multidisciplinary team, including speech pathology, is needed to enable the person to live well with Parkinson¿s disease and to prevent aspiration pneumonia, a leading cause of death in Parkinson¿s disease. A vignette prepared in collaboration with a person living with Parkinson¿s disease and his wife provides an ¿insider perspective¿ of the pervasive impact of difficulties with communication and swallowing.

Aims and objectives: The purpose of this study was to develop a way for nurses to understand how they negotiate and work with families in the intensive care unit (ICU). Background: The importance of family to the critically ill patient is described in previous research; however, research exploring how to work with families in the clinical context of ICU is modest and there is little information and research to understand how the nurse might work to integrate family and understand family needs in the clinical context. Design: The study was designed using action research methodology where an action research group of registered nurses from ICU proposed changes to how the family are defined, assessed and understood. Methods: In the action research group and reflective conversations, the group planned new assessment tools to use in ICU to work with the family. Results: Exploring their pragmatic knowledge of the context, the action research group suggested ways of working with families that did not impede the clinical work of the ICU. Families provided information about themselves and the patient that helped the nurse to plan care that redressed some of the asymmetrical nature of the relationship in the ICU. Conclusions: This research created novel ways of viewing family and offers strategies to address asymmetry between families and ICU professionals. Relevance to clinical practice: Positive steps to redress asymmetrical relationships can help ensure that family care is better integrated into ICU practice.
