Completing the Form

The Appointment - Job Details Form can be completed on-line before printing however the form cannot be dispatched electronically as you will have to sign it. If you fill the form in by hand please print clearly in block letters.

The form is to be completed by the Head of the Organisational Unit for fixed-term and casual appointments and forwarded to the Finance Manager for funds approval.

Other Documentation Required

If this is a new appointment to the University, the following forms should be completed by the appointee and attached:

  • Recruitment - Personal Details Form
  • Tax File Declaration Form - An original Tax File Number Declaration form (NAT 3092) can be obtained from HR Services, the Tax Office or selected newsagents.
  • Withholding Declaration Form - should also be completed if the staff member is claiming a family tax benefit through the tax system or claiming a zone, dependant spouse or special rebate.
  • The Workload Allocation Form should be completed for all academic staff appointments.
  • A complete resume, including copies of qualifications, if one is not already held in Human Resource Services.
  • A Position Description for general staff appointments.
  • A Superannuation Application Form is to be completed for new fixed-term and continuing appointments (not required for casual appointments) to the University. Contact your Human Resource Officer to obtain the appropriate superannuation booklet, with application form enclosed. Please note the commencement of salary payments cannot occur until the completed application form is returned to Human Resource Services.
 Notes: You will need to download Adobe Acrobat Reader to link to the Withholding Declaration Form. 
  If the Appointment - Job Details Form is being completed as an extension/continuation of an existing employment agreement, then ONLY the Appointment - Job Details Form is required.

Timeframe for Commencement of Salary Payments

The Appointment - Job Details Form must be received by Human Resource Services by noon on the Friday immediately after payday in order to be processed in the next pay. Staff should not commence duty until approval, that is, a copy of the staff member's confirmation letter from Human Resource Services, is received.

Should an occasion arise where a staff member commences prior to the confirmation letter from Human Resource Services is received, the organisational unit must ensure that TIMESHEETS ARE SUBMITTED if required. Please note timesheets are required for all casual staff, fixed term appointments that are less than 6 weeks in duration and all fixed-term ELICOS Teachers.

Period of Appointment

Short term appointments can cover full-time fixed term, part-time fixed term or casual staff but are usually for periods of up to 12 months (academic and research positions) and 6 months (for all other positions).
