Raising Issues

If you are having an issue with a Supplier of either Goods or Services and you require assistance THEN WE WOULD LIKE TO KNOW ABOUT IT.

Click the link below to lodge an issue by completing the on-line form. Simply follow the prompts to complete the form.

Contract Issue Lodgement Form

Once you have completed the Issue form, submit form.

The Issue form generates an e-mail that goes to the Contract Management Unit mailbox for action.

Issues are then entered into the Contract Management Issue Log.

Following investigation by the Contract Management Unit a response will be provided on the issue via return e-mail.

Please note - One issue per form.
