Please note: Application forms are continually updated. It is therefore advisable to download a new form for each submission.

The following questions are designed to assist you to select the correct form to use.
If you use the incorrect form, your application will be returned.

Application for NEW approval, or RENEWAL of approval:
1. Is this an application for a NEW project (ie. has not had previous animal ethics approval)?
YES	If the application is for a RESEARCH project:		Application form Animal Details	Guidelines	Attachments and SOPs
	If the application is for a TEACHING project:		Application form Animal Details	Guidelines	Attachments and SOPs
	If the application is for a research conducted in the FIELD, or involves WILDLIFE SURVEYS:		Application form Animal Details	Guidelines	Attachments and SOPs
NO	Go to Question 2.
2. For continuing projects: What type of application did you submit LAST year?

Repeat Initial
1^st Renewal
