Can Meet Needs
Skills Training to Support Needs Based Cancer Care (Progressive Disease)
Creating a more transparent and objective mechanism for deciding who accesses (or does not need to access) specialist palliative care services can ensure that the care offered to everyone with a life-limiting illness will more equitably address the needs of the tens of thousands of Australians who face this path every year.
Our three key messages:
- Using the NAT: PD-C routinely can better match patient and caregiver needs with the most appropriate people or services to address those needs.
- Use the NAT: PD-C in general practice, oncology, community and specialist palliative care settings.
- The NAT: PD-C can facilitate communication between primary and specialist care providers.
