Career Summary
Biography
Associate Professor Janet Wallace is a registered Oral Health Therapist with qualifications in dental therapy, dental hygiene, business management, tertiary teaching and a PhD in Oral Health. Janet has extensive clinical and oral health management experience in private and public dental practice and spent many years working for Northern Sydney, Central Coast, Area Health Service in various roles including; Manager Child Dental Services, Community Dental Health Programs Officer, Health Promotions Manager and Acting Director Community Dental Services.
In 2008, Janet joined the University of Newcastle as a Conjoint Lecturer in Oral Health to develop and implement the 'Aged care oral health student placement program in residential aged care facilities'. This was a new Australian initiative and the focus of her PhD.
In 2009, Janet was appointed to Lecturer in Oral Health and joined A/Professor Fiona Blinkhorn in writing the curriculum for the Graduate Diploma in Dental Therapy program. In 2014, she was appointed as Program Convenor for the Graduate Diploma in Dental Therapy program seeing the final year of offering of this program.
Janet was instrumental in identifying the need to redevelop the oral health program to include dental therapy and during 2014, Janet worked with Associate Professor Jane Taylor to write the curriculum for the new Bachelor of Oral Health Therapy program. This new program combined dental hygiene and dental therapy to offer a more relevant practitioner known as an 'Oral Health Therapist' with an adult scope of practice. In September 2015, Janet was appointed as the Program Convenor for the new Bachelor of Oral Health Therapy program. In February 2018, Janet was appointed as the Acting Head of Discipline for Oral Health.
In 2014, Janet translated her PhD research into practice by developing the ‘Senior Smiles preventive oral health care program for older people living in residential aged care facilities’. This pilot study was financed with a $100,000 grant from NSW, Medicare Local, Erina and placed a registered dental hygienist in five residential aged care facilities on the NSW, Central Coast. The dental hygienist provided oral health risk assessments, oral health care plans, oral health education and developed referral pathways for residents who required more complex dental care. This research has introduced a new model of oral hygiene care to residential aged care facilities and is changing the paradigm of oral health care in this environment.
Janet is a committed advocate for preventive oral health care for the elderly, is a keen Twitter user, is regularly quoted in media and has featured in several industry publications including the September 2015 issue of the Bite Magazine. She regularly provides guidance for dentists, oral health therapists and dental hygienists who wish to implement “Senior Smiles’ in their local communities and is focused on ensuring that ultimately ‘Senior Smiles’ is delivered in all Residential Aged Care Facilities Australia wide.
In 2016, Janet received a philanthropic grant of $540,000 from the ‘Elderslee Foundation Australia Ltd’ to extend her ‘Senior Smiles’ research. This significant grant supports an administrator to manage the program and placed qualified oral health therapists/dental hygienists in five major facilities on the NSW, Central Coast. This takes the ‘Senior Smiles’ model to the next level providing oral health care for a period of three years. This model proposes that at the completion of the three years, the participating facilities, will fund the program into the future and expand the service to all sister sites. Janet is committed to ensuring that ultimately, this model, becomes part of core business, in all Aged Care Facilities, Australia wide.
Janet is published in national and international journals, has presented her research at national and international conferences and has received a 'Faculty Research Staff Excellence' award, a 'Work Integrated Learning' award and a 'Research Higher Degree' award. Janet was a finalist in the 2017 Alumni Awards for exceptional community service, received a BRAWN Fellowship in 2017 and in 2017 was the only Oral Health Therapist to be awarded a Fellowship in the Academy of Dentistry International. She is a life member of the Australian Dental and Oral Health Therapists Association, an Honorary member of the Dental Hygienists Association Australia, a peer reviewer for the Australian and New Zealand Journal of Dental and Oral Health Therapy and an appointed Board Member of the NSW, Dental Council, currently serving her third term of office.
Janet is enthusiastic about providing oral health students with new learning experiences and in 2016 she implemented international student placements in Cambodia. In addition to the international placements, Janet has partnered with Ungooroo, Aboriginal Corporation Singleton to establish the UON Oral Health student placement program at Ungooroo Aboriginal Corporation, Singleton.

Given the increase in the aging population, there should ... [more] Introduction: Public service provision is one of the keys to reducing inequalities in the utilization of dental services. Given the increase in the aging population, there should be a focus on older adults¿ oral health. However, this is often overlooked. Objectives: This study investigates the effectiveness of public services in reducing income-related inequalities in dental service utilization among older South Australians. Methods: A multiple counterfactual mediation analysis using the ratio of mediator probability weighting approach was used to explore the proposed mediation mechanism using a South Australian population of older adults (=65 y). The exposure variable in the analysis was income, and the mediators were concession cards and the last dental sector (public or private). The outcome variable was the time of last dental visit. Results: Half of the older adults with high income (=$40,000) owned a concession card, and 10% of those who attended public dental services belonged to this group. Interestingly, only 16.3% of the study participants had visited the public dental sector at their last dental appointment. Results showed a negligible indirect effect (odds ratio [OR], 0.99; 95% confidence interval [CI], 0.85¿1.05) and a significant direct effect (OR, 3.09; 95% CI, 2.24¿4.87). By changing the potential outcome distributions to the counterfactual exposure distributions and taking the mediators¿ distribution as a counterfactual exposure distribution, the odds of dental visits occurring before the past 12 mo approximately tripled for low-income compared to high-income individuals. Conclusion: Income inequalities were associated with relatively delayed dental visits in older South Australians, and provision of public services could not improve this pattern. This might happen due to inequitable access to concession cards and public services. A review of policies is required, including addressing income inequalities and implementing short-term approaches to improve service utilization patterns in older South Australians. Knowledge Transfer Statement: The findings of this study can enable policymakers for informed decision-making about the provision of public dental services for older Australians. This study emphasizes the importance of reviewing the current public dental services and subsidies and implementing short-term approaches to reduce income inequalities for older Australians.

We ... [more] This cross-sectional study assessed the oral and physical factors contributing to improvement of the masticatory performance of community-dwelling older adults in South Korea. We enrolled 84 healthy older adults (38 men, 46 women; age, 71.40 ± 5.15 years) and assessed their skeletal muscle mass index (SMI), functional tooth units (FTUs), and mixing ability index (MAI). Associations between variables were analyzed using Spearman¿s correlation coefficient, and the effects of SMI and FTUs on the MAI were evaluated through linear multiple regression. FTUs were positively associated with the MAI in men and women (r = 0.339, p = 0.038 and r = 0.461, p = 0.001, respectively). SMI and FTUs were moderately associated in men (r = 0.459, p = 0.004). MAI showed an approximately 4.4 times increase for each FTU in men (B = 4.442, p = 0.037); however, after the SMI was added, this effect was no longer significant. In women, the MAI increased by about 6.7 times with each FTU (B = 6.685, p = 0.004). FTUs had a significant effect on the MAI only in women with low muscle mass. While there was no significant effect of the SMI on the MAI, its influence should not be overlooked.

Analyses from the 2017 Global Burden of Diseases, Injuries, and Risk Factors Study forecasted a continuation of global ageing throughout the remainder of the 21st century, creating major challenges for health-care systems to ensure healthy longevity for ageing societies. Oral health is an intrinsic constituent of general health and wellbeing; however, oral health is largely overlooked on the global health agenda. Oral conditions are mostly preventable or treatable, yet older people often do not receive the necessary routine care to maintain a good standard of oral health. The neglect of oral health constitutes a failure of global health policy and a failure to deliver the basic human rights of older people. The aim of this Personal View is to encourage a refreshed vision of oral health, enabling policy makers to recognise the implications of poor oral health in older adults. We call for urgent action to manage the projected challenges throughout the coming decades, to ensure that additional years of life are spent in a state of good health and to help mark global ageing, not as a burden, but as a major anthropological achievement.

[more] Introduction: The aim of this qualitative study was to measure the effect of a specifically designed orientation re-enactment DVD used to facilitate dental hygiene students transition from the classroom to a Residential Aged Care Facility (RACF) service-learning placement with less personal anxiety and more confidence in their role during the placement. Methodology: Final year students (n¿=¿47) were randomly allocated to one of 17 RACFs on the NSW, Central Coast, Australia. All students were then randomly allocated to a two-group study with the active group assigned to view the DVD prior to their placement. Students who viewed the DVD were asked not to discuss the content with students who were assigned to the control group. Post-placement focus groups were organized, recorded and transcribed verbatim. Data were collated, analysed and unitized into emergent themes. Representative quotes are presented in the results. The study was informed by 4¿years of previous quantitative and qualitative process evaluation of the RACF programme. Results: Focus group discussions identified that those students who had seen the DVD reported a shorter timeframe to successfully transition from the classroom to the RACF and stated that the DVD provided them with a realistic expectation of the RACF environment and their role in the placement experience. Conclusion: The orientation DVD reduced student anxiety and improved student confidence in their role during the placement by providing a realistic orientation of the RACF environment.

Methods: A qualified dental hygienist undertook a 24-week oral hygiene intervention in five residential aged care facilities to test the Senior Smiles, oral health model of care. The facilities were invited to take part in the research, which was funded by a grant from NSW Medicare Local, Erina. Residents were asked to consent to having oral health risk assessments, oral healthcare plans and to receiving referrals for treatment where needed. Pre- and post-intervention plaque scores were recorded for residents and P values calculated using a paired t-test. In addition, the number of residents examined, treated and referred for more complex dental care was recorded. Results: The statistical analysis program, SPSS, was used to conduct a paired t-test to compare pre- and post-intervention plaque scores on residents from the 5 RACFs. A statistically significant result of P < 0.0001 showed the intervention of the dental hygienist was effective in reducing plaque scores in residents across the 5 RACFs. Conclusion: The Senior Smiles model of care provided residents with preventive oral hygiene care, referral pathways for complex dental treatment needs and established a formal management programme for ongoing oral health care within the RACFs. The Senior Smiles model of care is successful and transportable.

[more] Aims: The aim of this study was to determine whether dental hygiene students attending residential aged care facilities (RACFs) during a placement programme gained any knowledge about the oral care of elderly patients and the RACF environment. Location: Aged Care Facilities on the Central Coast of New South Wales, Australia. Methodology: Final year dental hygiene students undertook a 12-week placement, one day per week, in one of 17 residential aged care facilities on the NSW Central Coast. They were asked to complete pre-placement and post-placement questionnaires, which recorded their knowledge of medical, dental and environmental issues related to older people. The placement questionnaires used five point Likert scales, ranging from strongly agree to strongly disagree, pre- and post-mean scores were produced for each question and P values calculated using a paired t-test. Results: Thirty-three students attended the placement, 26 (79%) completed both the pre- and post-placement questionnaires. Post-placement mean scores as compared to pre-placement mean scores showed significant improvement in student knowledge of medical (P < 0.05) and dental (P < 0.05) conditions specific to the older person and improvement in knowledge (P < 0.05) about the residential aged care facility environment. Conclusion: The placement programme enhanced student knowledge across three subject categories; medical and dental conditions of the older person and the structure and services of the residential aged care environment. © 2014 John Wiley & Sons A/S.

Measuring food and nutrition in older Australians$2,050
This study would identify the link between diet and oral health for older people who require assistance with their daily care. It would add a clinical tool that could be used by individual support workers in the aged care industry to ensure early detection of nutritional and oral health conditions.

Senior Smiles Preventive Oral Health Care for Older People Living in Residential Aged Care Facilities 2009 -
The Senior Smiles Preventive Oral Health Care for older people living in Residential Aged Care Facilities was supported by a philanthropic grant of $540,000 from the Elderslee Foundation Australia Ltd. The funding over 3 years from September 2017 to September 2020 enabled an administrator to manage the Senior Smiles program and placed qualified Oral Health Therapists/Dental Hygienists in 5 major Residential Aged Care Facilities on the NSW Central Coast.
This model of preventive oral health care is changing the way oral health is managed in Residential Aged Care Facilities by including oral health prevention, education and referral pathways to local dentists and dental prosthetists to provide holistic care for frail and elderly.
Senior Smiles is translational research that is operational in Residential Aged Care Facilities in Sydney, Newcastle and the Central Coast. There is interest in the model Australia wide and a training program has been written to provide oral health practitioners with the skills and knowledge to work in this environment. Senior Smiles has developed a reputation within the aged care industry and is being embraced by Residential Aged Care Facilities.
The Senior Smiles program has received numerous awards and A/Prof Janet Wallace, the architect and founder of Senior Smiles recently gave evidence about the program at the Royal Commission into Aged Care.
Government funding is required to support a national implementation of this program.

Senior smiles shine with spotlight on oral health
Oral health researchers from the University of Newcastle (UON) have been successful in securing more than half a million dollars to advance a program dedicated to preventive oral health care for the elderly.
