This course is designed for students from all health disciplines to advance their understanding of contemporary issues in chronic disease management. Students will explore contemporary and integrated disease models of care, including patient-professional partnership, collaborative care and self- management education. Principles of prevention and screening for common diseases in primary health care with emphasis on cultural awareness and safety will be applied and supported through identification and discussion of the rapidly emerging electronic and mobile support technologies.
| Objectives | At successful completion of this course students will demonstrate an advanced understanding of 1. Integrated disease models of care and primary care partnerships 2. Contemporary models of chronic disease management including patient-professional partnership, collaborative care and self-management education 3. The principles of prevention and screening for common diseases in primary health care including risk factor management and primary health issues 4. Interventions at individual level for common primary health issues such as smoking, diet, alcohol and physical activity 5. The centrality of family, country and kinship ties for Indigenous Australians and cultural influence on health behaviour 6. Sound knowledge of strategies for creating coordinated models of planned care based on evidence of best practice and care pathways for selected chronic diseases 7. The clinical application of emerging E and M technologies in primary health care. |
